Instructions:
Fill out this form to make an administrative request for a CIM site or CIM Site upgrade. You will be contacted shortly to confirm the information entered here. At that time, a Site Coordinator will be assigned to your project.

If you are requesting a New CIM site, or a Graphics Upgrade, the next step is to fill out the Site Request Step 2: Site Design form with your Site Coordinator.
